Rib Cartilage for Rhinoplasty

A large portion of the nose structure is cartilage. The nasal bone is the supportive structure for the top portion of the nose, but the lower bridge, nasal tip and nostrils are all made from cartilage. When cartilage is damaged from injury, disease, rhinoplasty or other factors, the nose structure can be compromised. To restore the form and function of the nose, cartilage grafting may be required to repair damaged cartilage. Weakened cartilage in the nose can cause aesthetic and functional problems. A common effect is nasal valve collapse, which can result in nasal obstruction and breathing problems. Other possible issues include a pinched nose or saddle nose deformity. The septum can also be damaged or perforated from injury or other factors, requiring repair.

When performing rhinoplasty with cartilage repair, cartilage grafts are needed to provide support. The most desirable cartilage to harvest for rhinoplasty is from the septum, but this is not always an option. If the septum is not a viable option for providing cartilage, rib cartilage is the best alternative.

Autologous Rib Cartilage Grafts

Rib cartilage can be obtained from the rhinoplasty patient, referred to as autologous cartilage grafts. This is the ideal source of cartilage, as there is plenty of cartilage in the ribcage and, it is straight for easy sculpting. Autologous rib cartilage reduces risk of rejection and other complications associated with donor cartilage grafts. There are other cartilage graft options such as harvesting ear cartilage or using donor rib cartilage, but these are not preferred over autologous rib cartilage.

To harvest rib cartilage, a separate procedure is performed before the rhinoplasty. A small incision is made to retrieve cartilage from the ribs, which is easily concealed once it heals. The cartilage is carefully carved to create the needed graft material to add support to the nose structure.
